• Entries must be submitted on this official entry form with at least 25 categories filled out. Ballots must include name, address and telephone number to be counted. Ballots are available at The Decatur Daily, 201 1st Avenue, SE, Decatur.
• 1 Entry per person. No purchase required. Entries must be received by 5:00 pm on Friday, Sept. 19, 2014 to be counted. No photocopied entries allowed unless provided by The Decatur Daily. Only 1 ballot per person will be counted. Ballots which are illegible or not containing name, phone number and address WILL NOT be counted.
• Category winners will be announced in a Decatur Daily special section Thursday, October 30, 2014. Winners in each category are determined by reader votes only.
• Cash winner will be chosen by a random drawing of all entries. Employees of The Decatur Daily & their immediate family are not eligible to win the $250 cash prize.
